New Patients

New patients need to complete a patient registration form prior to their appointment. The forms can be printed and completed prior to the visit, or patients should plan to arrive 30 minutes prior to the start of the appointment to complete the forms.

Vibrant Health accepts most insurance plans. If you have a co-pay, please be prepared to pay at the time of the visit. Please be advised that patients are responsible for verifying if our providers are in-network and the co-pay amount.

Vibrant Health offers high-quality health care at a reduced cost by offering a Sliding Fee Program. In order to apply for the discount, a completed Sliding Fee Program Application is required along with a photo ID, proof of address and proof of income for all household members.

The form can be printed and completed prior to the visit, or at the clinic before a visit.
